The Phylactery of Kel'Thuzad is a quest item dropped each time Kel'Thuzad is killed in Naxxramas (similar to the 
[Head of Onyxia], 
[Heart of Hakkar], etc.).
Rewards
There's a choice of two rewards from the quest 
[60R] The Fall of Kel'Thuzad, one trinket for physical damage and one for magic damage.
